Eligibility

Eligible Employees:

  • Employees scheduled to work 30 hours or more per week are considered full-time.
  • All bi-weekly and weekly paid employees are eligible for benefits on the first day of the month following 60 continuous days of continuous service.

 Eligible Dependents*:

  • Your legal spouse or domestic partner
  • Your children up to age 26 (including biological children, legally adopted children, stepchildren, foster children, and children for whom you have been court appointed legal guardian)
  • Your children over age 26 who are dependent on you for financial support and are not able to support and care for themselves due to a physical or mental disability

If adding a domestic partner or child(ren) of a domestic partner to your medical, dental, and/or vision plan, it is important to understand that your portion of coverage will be deducted from your paycheck as pre-tax, and your domestic partner and domestic partner’s children’s coverage is deducted as post-tax. This will be reflected on your paycheck as “imputed income”. A Domestic Partner Affidavit is also required before coverage begins.
 
*NOTE: All dependents must be verified before coverage can begin. Please have birth certificates, marriage licenses and/or prior year tax return documentation.
